This is a valuable resource that I created for the special education team within my building. It’s a Google Form template
specifically designed for obtaining parent input for Individualized Education Programs (IEP) for our special education students. The purpose of this form is to establish better collaboration between the special education team and the
families of our students.
The Google Form consists of a series of questions that are aimed at gaining the parent’s input in the development of the IEP. By involving the parents in this process, we can ensure that their perspectives, insights, and concerns are taken into consideration. This will ultimately lead to more effective and comprehensive plans for our students.
Additionally, this product includes an editable email template
that you can personalize and send to parents. This email provides a brief
introduction to the purpose of the Google Form and outlines the importance of
collaboration between the IEP team and the families. The email also includes a
place to insert the link to the Google Form for easy access.
