The questionnaire has 19 questions in 4 sections:
* The Student’s Background
* The Student’s Performance in Class
* The Teacher’s Experience and Training in Working with ELLs
* Modifications Being Made

Teachers, coaches, counselors, school psychologists, and administrators can use this questionnaire to obtain information about multilingual students who are not doing well in school.ย 

To use this tool, first an educator answers the questions on the questionnaire.ย  Then, the educator should read the annotated questionnaire to get insight into how the studentโ€™s profile correlates to the issues raised in the questionnaire.ย  Alternately, an educator can read the annotated questionnaire first and then decide whether or not to proceed with filling out the blank questionnaire.ย  After that, and combined with data obtained from other sources, educators will be able to make a more informed decision about how to best support the Multilingual Learners of English they work with.

The questionnaire was developed for teachers at a K-8 school and can also be used at the high school level.ย  It was presented at many in-service teacher trainings for regular ed and special education teachers, who all said it was a very useful tool. ย 

Using this questionnaire can help all educators begin the process of determining whether a struggling multilingual student needs special education services, ESL support if it is not being provided, or additional language support if the student is already in an ESL program.
