Description
The purpose of this zip folder is to provide all of the tools and resources necessary to start up an effective SIT (Student Intervention Team) process in your school. Having an effective SIT team in your building is critical to helping support students and teachers be successful as well as manage appropriate referrals to Special Education. SIT Process 101
ยท This file details the process teachers should follow when making a referral to the SIT team. There are links to other documents, listed above, and at my school, we have all of these documents in Google Drive, so they automatically pull up when you click on the link.
SIT Referral Template
ยท Teachers are responsible for filling out this formโs light grey shaded sections prior to each monthโs SIT meetings. I usually send an email out two weeks prior letting teachers know they have one week left to get their forms filled out to be on the agenda for our next meeting. One week prior to the meetings, I email out to the team with a list of students we will be meeting on so they can review the referrals ahead of time and can come to the meeting prepared.
Parent SIT Notification Form
ยท This is a form email that can be used by teachers to notify parents when their child is being referred to SIT as well as the rationale for it. I expect the classroom teachers to follow up with the parent following the meetings to let them know what interventions are being put in place to help support their child.
Caregiver Input Inventory
ยท This form is also sent with the form email above so that parents can provide input on their child. This file is scanned and kept in that childโs file for SIT. We refer to it during the meetings.
Gifted and OT Referral Forms
ยท If you would like to have a related service provider from special education do an observation, these forms can be filled out prior to their observation. From there, the related service provider may recommend a full evaluation or some general education interventions that can be done in the classroom to help support that student.
Team Meeting Structure
ยท In my experience, meetings can be taken over with complaining or by focusing too much on one child. This structure allows each student to receive the same amount of time, focus, and energy.
Other Forms
ยท The remaining forms are examples of data collection that can be done for behavior or other SEL goals. The data collection ideas document provides examples of both behavioral and academic data that might be collected.
Following a referral, the SIT team should meet on students to follow up during follow up the next month, or as needed moving forward.
