MTSS/SST Intervention Team Student Tracker | For Teams & Interventionists

Keep student concerns, intervention notes, support information, and team discussions organized in one easy-to-use digital tracker.

The MTSS/SST Intervention Team Student Tracker is a practical Google Sheets™ resource designed for MTSS/SST teams, interventionists, intervention coordinators, and student support staff who need an efficient way to track students who have been brought to their attention.

Whether a teacher has raised a concern, a student is receiving additional support, or your team simply wants to monitor a student over time, this tracker provides one centralized place to document important information and maintain a clear history of team discussions.

No more searching through meeting notes, emails, handwritten lists, or multiple documents to remember who you discussed, why they were brought to the team, and what has happened since.

What This Tracker Helps You Do

Use the tracker to:

Keep a centralized list of students being discussed or monitored
Record student name, grade, teacher, and support status
Identify students with an IEP, 504 Plan, or Monitor status
Document concerns, observations, data, updates, and team notes over time
Maintain a running history of student support discussions
Organize information before MTSS/SST meetings
Quickly access an individual student’s information
Generate clean, organized student PDF reports for meetings, referrals, documentation, and team planning

Designed for Monthly Intervention Team Meetings

This resource is especially helpful for schools that have a regular MTSS, SST, intervention, or student support team meeting.

Enter each student once on the Student Input sheet and continue adding dated notes, concerns, data, and updates as the year progresses.

Individual student tabs organize that information into an easy-to-read student record, making it simple to review a student’s history during team meetings without scrolling through a large master spreadsheet.

Built-In Features

The tracker includes:

Student Input dashboard for centralized data entry
Individual student information pages
Up to 20 dated notes/updates per student
Support-status tracking
Automated student information transfer
Custom menu for creating additional student tabs
One-click student PDF reports
Portrait PDF reports formatted for easy printing and sharing
Organized documentation for ongoing intervention and support discussions

Great for Teams — or Individual Interventionists

You don’t need a formal intervention team to use this resource.

It can be used collaboratively by an MTSS/SST or student support team, or independently by an interventionist, intervention coordinator, instructional coach, counselor, administrator, or other student support professional who needs a consistent system for keeping track of student concerns and follow-up.

Spend Less Time Organizing. More Time Supporting Students.

When multiple students are discussed month after month, it’s easy for important details and previous conversations to get lost.

This tracker creates a running student support history, helping you quickly answer questions such as:

Why was this student originally brought to our attention?

What concerns have been documented?

What has the team already discussed?

What new information or data has been collected?

What should we follow up on next?

Everything stays organized in one place so your team can walk into meetings prepared and spend more time making informed decisions about student support.
